    <P><A href="http://www.asfa.org.au/" target=_blank>The&nbsp;Aviation Safety Foundation Australasia </A>is calling for the development of a national air safety strategy in Australia following the release of the<A href="http://www.flightglobal.com/articles/2006/12/05/210948/transair-grounded-as-likely-air-safety-risk.html" target=_blank>&nbsp;final report into the May 2005 Lockhart River crash</A>. It believes the accident highlights that lessons have still to be learned from a similar Monarch Airlines incident in 1993.</P> <P>The Transair Fairchild Metro 23 crashed into terrain on approach to Lockhart River aerodrome, Queensland, killing all 13 passengers and two crew on board. The Australian Transport Safety Bureau identified 19 factors that contributed to the crash, including pilot error, deficiencies with the operator and oversight failures by the regulator, the Civil Aviation Safety Authority.</P> <P>The Monarch Airlines' Piper Navajo Chieftain crashed into terrain on approach tohttp://www.flightglobal.com/news/articles/Australian-national-strategy-urged-213394/

    <body lang=EN-GB style='tab-interval:36.0pt'> <div class=Section1> <p class=MsoNormal>A top-level shakeup has been confirmed at New Zealand’s CAA in the wake of a critical coroner’s report into a fatal general aviation aircraft crash.</p> <p class=MsoNormal>The Government says in a statement that it has accepted the resignation of CAA chairman Ron Tannock. Earlier this week director general of civil aviation John Jones announced his resignation.</p> <p class=MsoNormal>Transport minister Annette King says that in his resignation letter Tannock, who has been CAA chairman since 2004, said “he felt it was essential that a new chairperson be in place to launch and manage the recruitment of a new director of civil aviation”.</p> <p class=MsoNormal>King adds: “The last few years have been difficult with the Civil Aviation Authority having to deal with a number of contentious and high profile issues.
