Aircraft continued 6hr flight to Tokyo and landed safely. RA No. 1 engine hit a cable on approach to LAX, severing hydraulic lines to landing gear. Slid off runway into dirt embankment. T/O Aborted take-off and veered on to another runway after one engine suffered mechanical failure. T/O Sheared off the roof of an airport vehicle during take-off. Vehicle had been cleared on to runway simul taneously. L Nose landing gear collapsed on runway. T/O Stage one high pressure turbine disc in No. 2 engine broke up during take off run. Aircraft aborted take-off. Serious fire started as a result of turbine fragments penetrating fuel tanks and lines, but all on board were evacuated. Aircraft a write-off. C Both engines failed during early climb after water-methanol take-off. JP4 fuel was later found in the WM tanks. ER Emergency wheels-up landing made after LP impeller failure caused in flight fire. L Landed with nose gear half down. An hydraulic valve had been fitted in reverse. L Damaged during heavy landing. L Overran runway by 100m in heavy rain. Gear and one engine damaged. T/O Lost power and stalled shortly after take-off. Caught fire on impact. L Overran runway. ER All four engines flamed out when air craft (at night) entered a cloud of volcanic dust at 38,000ft. Was carrying 4,000lb cargo. Crew rescued. L No 4 engine pod contacted runway during landing. G Towing incident: tow bar broke and aircraft continued moving forward, striking back of tug and tearing engine from wing. L Port main gearcollapsed during landing. ER Starter port engine disintegrated caus ing fire. Cowlings torn and burnt and electrical wiring damaged. ? Main gear collapsed and aircraft swung off runway.
