AA AA ER ? Circumstances Fan blade failure in No. 1 engine, near top of climb. Crew interpreted cockpit data as showing a No.2 engine failure and shut it down. No.l engine suffered serious mechanical failure on final approach. Aircraft climbing through 20,000ft out of Honolulu when forward star board underfloor cargo door blew out, taking a large section of fuse lage above it. Nine passengers were blown out of the hole, and the No.3 engine suffered serious foreign object damage. Crashed soon after take-off in snow. Crashed on approach in 3,000ft visibility. Failed to get airborne because elevators jammed or locked. Overran runway, killing one person on ground as well as 16 on board. Circumstances Failed to arrive at destination. The flight was a regular service for remote civilian communities carried out by the military. The aircraft force-landed and overturned. Circumstances Hit tree immediately after take-off. Possible engine failure. Hit trees 3,000ft short of runway while on ILS approach. Crashed short of runway. Aircraft appeared to stall, and then crashed. Initial report suggests CG was too far aft because of substantial weight disparity between passengers. Reported engine trouble soon after take-off, lost height, and hit a house. Crashed on landing. Bad weather. Circumstances Lost height after take-off. Right wingtip and propeller hit ground. Con tinued flight, hitting 20ft tree tops, then crashed. Aborted first take-off, and on second banked sharply just after rotation, and crashed into lagoon. Aircraft operated by Intercargo. Hit high ground 8 miles south of Subang International Airport. Weather bad, no apparent aircraft problems. Hit high ground 17 miles from airport. Crashed" 1 mile from airport following take-off. Turned back on receiving cargo door warning soon after take-off. Air craft crashedshort of airport. Explosive cargo. Crashed Into buildings 1 mile short of runway. On the ground 17 were killed and 200 injured. Crashed soorirafter take-off, killing two people on the ground. Crashed 5 miles from De Kalb Airport, cause unknown. Tree spray operation.
