Crashed on approach. RA Crashed into sea about 15km out on night approach to runway 34 at Kinsgford Smith Airport, Sydney. No emergency call. RA On approach to Nalchik, aircraft pitched up abruptly, then down, and control was lost. Aircraft crashed short of runway. Icing suspected. Flight, from St Petersburg via Volgograd, carried a cargo of coins. ER Three engines flamed out in severe icing conditions, although one was re-started Crashed on forced landing at night with two port engines operating. TO Just after take-off from a touch-and-go landing on a training flight, a nose-high atti tude was adopted by the aircraft. Then the left wing dropped and the aircraft turned sharply left, crashing near the international terminal parking ramp. Burning wreck age hit an Aeroflot Russian International llyushin II-86 on turnaround maintenance, setting it on fire. Eight people died, including three engineers on the li-86. AA Cargo flight. Flew into the ground about 15km north of Tamworth Airport while carrying out instrument descent. TO Shortly after take-off from runway 21, the pilot advised air traffic control that he had shut down No.2 engine. The aircraft crashed about 1km from departure-end of run way. Cargo flight bound for Portland. L Severe pitch-up during Cat III automatic landing, with aft centre of gravity, just as full reverse thrust and spoilers were deployed. Autopilot disconnected man ually, and full-forward stick deflection was required to put nose down. Similar incident involving A320 G-BUSJ at Luxembourg again on 3 Feb, 1994. ? 140 L Landing in icy rain, aircraft skidded off runway into deep mud. 18 246 L No.1 engine partially separated from the wing during the landing roll and scraped along the ground. A forward fuse pin was broken, aft one missing. 4 110 TO Aborted take-off on runway 13 because of slow acceleration in snow, but over ran runway. ? ? L Aircraft landed "positively", but bounced because spoilers — which were armed — did not deploy because throttles had not been retarded to idle. Second touchdown registered 1.4g because throttles were retarded during 3s bounce, and wheels had also spun-up from first touchdown, so spoilers de ployed. Gear was slightly damaged. 6 110 RA On an ILS approach in poor visibility, the aircraft dipped below the glideslope and the left wing hit an approach-light stanchion. Then the aircraft touched down hard 50m short of the runway threshold, causing gear failure. The air craft slid 600m, groundlooped and burst into flames. L Aircraft en route Newark-Fort Myers, but was diverted to Orlando when port main gear would not extend fully. ER During a climbing flight-level change in autopilot, the aircraft rapidly pitched up and speed decayed until buffet. Autopilot was disconnected and pitch atti tude reduced "with great difficulty". ? ? AA Emergency declared in descent and No.3 engine shut down with low oil pres sure and contents. No.2 engine then showed intermittent low oil pressure during final approach and was shut down on touchdown. ? ? RA Severe turbulence on approach to runway 13 caused flaps to lock in the select ed fully-down position (position 4). Turbulence as a cause of flap-lock had not been foreseen, so the central screen advice to pilots was inappropriate and, when carried out, left the aircraft highly sensitive in roll [Flight International, 29 June-5 July, P8). 17 80 ER Inadvertent manual disengagement of autopilot caused pitch oscillation and altitude loss, according to pilot report. Aircraft suffered minor damage. Engine tail-cone separated over UK south coast at Lewes and rudder damper rivets failed. Gear extended by free-fall.
