ER Passenger-operated compact-disc player caused interference on all VHF radios. 4 21 C At 200ft, after take-off from runway 16 at Sydney Kingsford Smith Airport, the port engine lost power so was shut down and feathered. Aircraft would not maintain height, so pilot carried out a controlled ditching in Botany Bay. The aircraft could not maintain height on a single engine, so pilot force-landed the aircraft in a paddy field. Dipped below approach path on VOR/DME approach for Athens runway 15: in IMC. Hit power lines on ridge, suffering damage and single engine failure, but continued to land. Aircraft veered off runway during take-off. Snow on runway was unevenly cleared and hid runway edge lights. Scheduled night flight. Automatic fuel cut-off stopped both engines when turbines oversped. It is believed that, on entering descent for Baton Rouge, crew selected throttles aft of flight-idle into "Beta" range. Crew could not re start engines, so diverted for a glide landing at False River, where the aircraft overran the runway. 6 5 Overran runway in poor visibility. 4 37 G Starboard propeller destroyed when it struck a ground power unit. ? ? RA Landed 15m short of runway and tore off right main gear leg. 4 25 ER Engine failure, forced landing in fields. ? ? L Loss of control during landing roll. Aircraft left runway. ? ? L Inbound from Bogota, overran runway. 4 33 ER Fire warning in left engine. Fire drill completed and aircraft landed at Dallas Fort Worth. Fire damage found to bottom of engine cowling where a leak in the fuel nozzle drain collector was discovered. ? ? L Wingtip struck ground on landing. The aircraft came to rest upside down and caught fire. The aircraft halted safely on the runway, but a wheel fire had to be extinguished. RA Rapid descent began just before touch-down, causing hard landing and substantial damage. G Parked aircraft was destroyed by fire, which is believed to have started in a wheel- well. C Aircraft failed to pressurise on climb out of Yposilanti, Michigan, so crew elected to continue to cruise altitude 35,000ft using oxygen masks. The captain ill, so co-pilot took command and diverted to Covington, Kentucky. Captain was hospitalised and reported to be "in serious condition" on admission. L Chartered by the Colombian defence ministry. A mainwheel tyre burst on take-off the pilot elected to return for landing. Touchdown was more than half-way along the runway and the aircraft overran, even though the pilot retracted the gear to slow it further. Cargo included 70 boxes of dynamite. G After landing and taxiing to ramp, decision was made to re-position for parking. More taxiing to final stop caused brake fire which destroyed the aircraft. L Ferry flight. Nose gear farted to extend. Slight damage on landing. L Aiming to use all the short, narrow runway, the pilot touched down 2m short of the threshold and hit a drainage ditch.
