AIRLINE SAFETY REVIEW 24 June KHORSAir Antonov An-24 (UR-26197) Cafunfo Airport. Cafunfo. Angola 4 10 L Approach to land was carried out in a thunderstorm, during which the aircraft suffered severe turbulence and a lightning strike. On landing it overran the runway. 21 July Northern Air Cargo Douglas DC6A (N313RS) Nr Russian Mission, Alaska. USA 4 4 RA A fire in No 3 engine could not be extinguished on an Anchorage-Bethel flight, so the crew diverted to Russian Mission airstrip. Reports indicate that the crew had to go-around twice because of people or vehicles on therunway. Before the third approach could be made, the wing broke away. 12August Bradley Air Services DHC Twin Otter (C-GNDN) Markham Bay Airstrip. Baffin Island. Canada 2 2 C The aircraft bounced several times on landing, and the crew decided to attempt a go-around. On doing so. however, the aircraft appeared not to have climbed and the aircraft's undercarriage hit a rock, precipitating a crash into a rock. 14 August Air North Douglas DC-4 (OFGNI) Iskuit River. Branson Creek. Canada 1 3 C A fire started near No 2 engine and could not be extinguished. As the crew set out to return to Bronson Creek. No 2 engine separated from the wing, taking No 1 engine with it The captain successfully ditched the aircraft in the Iskuit River, but only the copilot and flight engineer managed to swim to the bank of the fast-flowing river. • 19 August SPAir llyushin 11-76 (RA76513) Belgrade Airport Yugoslavia 12 12 C Shortly after take-off for Malta, the captain advised ATCthathe appeared to be losing all electrical power, and with it all his instruments, communications and navigation aids. Having circled for about 2.5h.the aircraft came down 800m short of the runway on a night approach. 8 October Aeroflot-Russian International Airlines Antonov An-124 (RA82069) Turin Airport. Italy 2 23 C The crew abandoned a kxaliser-only ILS approach in light rain and 1.500m visibility because theyjuoged the approach to be tco high. Durm^ failedto climb and it hit buildirgs on the extendedcentreline about ll<m from the upwirtf end of the runway at a heigM of about 25m above air fi^ Two people on the ground were killed. 22 October MillonAir Boeing 707-323C (N751MA) Manta. Ecuador 4 4 C Just after takeoff from Manta. the aircraft appears to have suffered an engine fire. The aircraft came down in a residential area killing 30 people and seriously injuring about 60 more. 23 October LADE Undershot by about 600m on approach to runway 11. Boeing 707 (LV-LGP) Ezeiza Airport. Buenos Aires. Argentina. RA 12 November Chimkentavia Iryushin II-76TD (UN-76435) Nr New Delhi. India 37 37 AA The aircraft, operated by a unit of Air Kazakhstan, was positioning to New Delhi to pick up freight. Cleared to descend to 15.000ft inbound, it collided with a Saudi Arabian Airlines Boeing 747-100 outbound on the reciprocal track which had been cleared to climb to 14.000ft (See Fatal accidents, scheduled flights). The llyushin pilot had stated that he had levelled at 15.000ft and was advised to look for the Saudi Arabian aircraft 27 November Russian AF Iryushin II-76T Nr Abakan, Krasnoyarsk. Siberia. Russia 29 29 C CRT. Carrying out a commercial flight and carrying some 30t cargo as well as the large 'crew', the aircraft crashed into a mountainside soon after take-off from Abakan at 20:00. ___ 21 December Selva Antonov An-32 (HK-4008X) Nr Medellin. Colombia 4 4 AA Flying for Caricarga Colombia on a newspaper delivery flight froni Bogota. Appeared to go out of control sonie 15km Iran airport One of the wings was found separated from the m^ than lkm. 22 December Airborne Express Douglas DC863C (N827AK) Nr Narrows. Virginia, USA 6 6? CFTT. Crashed in mountains during a check flight after maintenance and modification. Weather conditions were very poor. Tennessee, USA 5 88 L The aircraft touched down short of the runway 02R threshold in a high nose-up attitude because the ground-spoilers had deployed inadvertently. The tail struck the ground between the runway-end and the first approacMighting bar, the main gear touched down 15m further on and the nose gear just before the runwayend. Impact with the runway destroyed the nosewheels and the aircraft bounced airborne. The captain carriedout a go-around and repositioned for an approach to runway 31, where a safe landing was made. After take-off from Atlanta, the ground-control relays had not transferred to flight mode and the gear would not retract so the crew had tripped the relay C-Bs. When it reset the C-Bs on approach, the spoilers deployed because they had been armed. 15 January Air India Boeing 747-200 (VT-ERJ) Birdstrike on No 4 engine, which was shut down. The aircraft returned to Mumbai. Mumbai. India -/- ?/? 21 January Royal Nepal Airlines Airbus Industrie A310-3O0 (DAPON) Dubai Airport. Dubai After pushback, the captain started to taxi upon receiving a signal from the ground engineer, but the aircraft's right engine hit a tug. -/- 10/150 23 January Alitalia Boeing 747-2006 (l-DEML) Miami International Airport. Florida. USA -/- 19/231 G Collided with a VASP McDonnell Douglas MD-11 (PP-SPE) while taxiing in good visibility at night. There were two separate slight collisions, one caused by the 747 and the other by the MD-11 as the aircraft was manoeuvred to comply with ATC instructions. 30 January Delta Air Lines Boeing 727 LaGuardia Airport. New York. USA -/• ?/? Turbine hub failure. 1 February ValuJet McDonnell Douglas DC-9-32 (N903VJ) Starboard mairUandinggear leg collapsed on landing. Nashville. Tennessee. USA •/- ?/? 14 February AVENSA McDonnell Douglas DC-9-51 (YV-87C) Nr Caracas. Venezuela -/- 6/85 C Climbing through 20.000ft en route CaracasSarquisimeto, the aircraft suffered sudden cabin decompression. The upper portion of the rear pressure bulkhead had broken away because of a fatigue crack at the upper right comer of the aft door tothetopofthe bulkhead. The crew carried out normal drills and the aircraft landed safely back at Caracas. 19 February Continental Airlines McDonnell Douglas DC-9-32 (N10556) Houston Intercontinental Airport. Texas, USA Crew failed to select undercarriage down before landing. •/• 5/82 8 March Canadian Airlines International firm tailstrike on landing. Boeing 767-30OER (C-FOCA) Halifax. Nova Scotia. Canada ?/? 12 March Nigeria Airways Boeing 737-200 (5N-ANZ) Kano. Nigeria Engine fire shortly after takeoff for Lagos. Aircraft returned to Kano and landed safely. Fire caused substantial damage "in the turbine area". V- ?/? 2Apnl Philippine Airlines Boeing 737-300 (EIBZF) Manila International Airport. Philippines -/- 6/141 TO Collided with a Pacific Airways de Havilland Canada DHC-&200 Twin Otter, which was crossing the runway at night Collision occurred 200m from the beginning of the 737's takeoff run. The Twin Otter (RP-C1154), which had no passengers on board, was pushed some 130m before the aircraft both stopped. Its crew was unharmed. 6 April Cyprus Airways Airbus Industrie A310-200 (5B-DAR) Athens. Greece -/- Rear cowling detached from No 1 engine following uncontained compressor failure (General Electric CF&80). Engine fire was extinguished and aircraft landed safely. 30 May Virgin Atlantic Airways Boeing 747-400 (G-VHOT) Iqualurt Airport, North West Territories. Canada ?/? 21/376 TO Pilot diverted to Iqualuit because a passenger became seriously ill. During taxiing for the refuelling point, the aircraft "sNo4 engine struck the refuelling station suffering major damage. The pilot was taking guidance from three marshallers. 12 Jury America West Airbus Industrie A320 Columbus, Ohio. USA 7/37 Cabin depressurisation early in the climb. The crew diverted to Port Columbus. 14 June Bouraq Indonesia Airlines Boeing 737-200 (PK-IJJ) Jakarta. Indonesia -/- ?/? L Inspection established that a heavy landing had caused structural damage to the fuselage, but it was not established on which landing the damage had been caused. 20 June TransAero Boeir^ 757 (EHXU) Ekaterinburg. Russia -/- 9/66 T Starboard maingear bogey beam fracture near the pivot produced an undercarriage fire. While being towed to the stand .the port mam gem (wgey beam also was found to have fractured at the same point. 14 August Delta Air Lines Boeing 727 LaGuardia Airport. New York. USA "J- 7/7 C~ Engine surge followed by uncontained turbinehub failure of one of the aircraft's outer Pratt & Whitney JT8D-15A engines. 21 August Egyptair Boeing 707-320C (SIMVX) Ataturk Airport. Istanbul. Turkey Aircraft overran runway by 200m on landing, destroying its landing gear. Just before touchdown, the aircraft encountered a heavy rain shower. •/- 8/120 19 October Delta Air Lines McDonnell Douglas MDS8 (N914DL) LaGuardia Airport. New York. USA -/- 5/58 RA Undershot on approach to runway 13. hitting approach lights andtheedge of the concrete deck, shearing off the mam gear.
