AIRLINE SAFETY REVIEW 28 October American Airlines McDonnell Douglas M&80 (N244A) LaGuardia Airport, New York, USA -/• 5/88 AA inbound to LaGuardia, the crew received warnings that the nose-gear was not locked down. After a fly-by inspection at LaGuardia. the crew diverted to JFK Airport. Landing on runway 31 there, the nose gear collapsed. 6 November Malaysia Airlines McDonnell Douglas MD-11 (N271WA)Ezeiza International Airport, Buenos Aires. Argentina -/- 15/268 On the second ILS approach to runway 11 the aircraft toucheddown long and fast on the wet runway and overran. The approaches were flown manually because of problems with the autopilots. 16 November Vietnam Airlines Nose gear collapsed on landing. TupolevTu-134(?) Da Nang, Vietnam ?/? 3 December Polynesian Airlines Boeing 737-300 (5W-ILF) Apia, Western Samoa veered off the runway after landing from an ILS approach. Had been chartered to carry a Brazilian football team. 3 May Penair Fairchild Metro III (N670PA) St George Island Airport, Alaska, USA Sink rate too high on approach, and action to arrest it was taken too late to avoid a hard touchdown which caused gear failure. 2/11 14 May Allegro Air McDonnell Douglas DC-9-14 (XA-SNR) Tampico, Mexico -/- 4/42 ER Using Omega navigation, the pilot became lost. He diverted to Tampico. but the aircraft ran out of fuel, both engines flaming-out with some 90km to go. The aircraft finally touched down some 300m short of the runway. 28 May Martinair Holland Boeing 767-3O0ER (PC-MCH) Of f US east coast 7 204 ER •partial DC power failure' caused total EFIS failure and robbed crew of flap and reverse-thrust options. Aircraft, originally bound for Miami, was diverted to Boston, USA, where the crew landed it safely in good VMC. Peru Right main gear collapsed on landing. 13 January Vanderhoof Flying Service Beech Baron 9&B55 (C-C-GXHN) Vanderhoof, British Columbia, Canada Nose gear failed to lock down, possibly because of frozen slush from takeoff run at Minaret. 25 January Transportes Aereos Neuquen Fairchild Metro III (LV-AOR) Rincon de los Sauces Airport, Neuquen. Argentina Touched down just short of the runway. Canada Struck a snow bank before touchdown. Impact tore off main gear and aircraft came to rest on runway. 2 April Prince Edward Air Cessna 208B Grand Caravan (C-FLAQ) Nain, Newfoundland, Canada Landed lOOm short in daylight, but in white-out conditions (snow). Pilot was trying to land precisely on the threshold because the runway is short. 7 April Island Air Gold Coast Pilatus Britten Norman Islander (VH-HIA) NrCurrumbin, Queensland, Australia flight engine lost power. Pilot diverted to follow the coast and, upon failing to maintain height, ditched just offshore. 7 April Dolphin Airlines de Havilland Canada Twin Otter (N143SA) Virgin Gorda. British Virgin Islands Veered sharply left, ran off runway and hit fence. 16 April Merpati Nusantara Fokker F27 (PK-MFW) El Tariq Airport, Kupang, Indonesia Port maiogear leg would not retract fully. Gear failed to deploy, so crew landed with gear up. Liberia Undershot runway by about 5m. which caused the left main gear and wing to fail. There was no fire. 23 September Vanua Air Charter Pilatus Islander 2A(DQ-FIF) Gau Airport. Fiji Overran on landing in poor visibility and rain, damaging landing gear. 8 December KLM CityHopper Fokker 50IPH-KVK) London Heathrow Airport, UK •/- -/- -/- -/- -/- Left main undercarriage failed to lock down.
