After starting a left turn, however, the aircraft spun into the ground. It was nighttime with good weather, temperature 32'C. airfield elevation 5.500ft. Crashed on a non-precision night approach in good weather following a 4h 30min flight from Lima. Peru. The aircraft had been cleared for a DME arc/localiser approach (13km arc) and had reported established on the arc. but crashed some 17km from the airfield. Main gear collapsed on landing. Gearfailed to extend desp!- c (• •••-.? «Ho land with gear up. Landed long in wet weather, overran and rolled down a slope. Veered off runway and hit embankment. Hard landing after aircraft ballooned at the flare and spoilers operated. Pilot had trouble controlling the aircraft in roll several times during the approach. Eventually he appears to have lost speed, stalled and lost control, the aircraft hitting the ground about 3km short of the runway threshold. There was a Boeing 757 on approach 9km ahead. Hit high ground. Pilot is believed to have reported s technical problem. Pilot forgot to lower the gear on a night landing. Collided with a Beech A36 which was formating on the DC-3 to take air-to- air photographs. The A36 also crashed, killing its two occupants. Just before starting to taxi, the right brake failed and the aircraft swung left. Attempts to engage reverse thrust failed and by the time the aircraft stopped it had hit and badly damaged two other aircraft. The aircraft was preparing to depart for a test flight after major modifications, including re- engineing with Pratt & Whitney PT6 engines. Anomalies were found in the brake maintenance and the throttle rigging for both engines. Landed with gear partially retracted. Cleared to land on a night approach to runway 31 with 20km to go. A truck carrying out maintenance on the runway lights 40min earlier had failed and could not clear the runway. The driver saw the approaching aircraft and warned the controller. An instruction to the pilots to go around came too late and they tried to steer clear, but hit it with the right wing. Left main gear collapsed during the landing roll. On an ILS in poor weather following a precautionary fuel diversion, the aircraft touched down 1.000m short of the runway, but suffered no damage. During the subsequent attempt to taxi to the runway, however, the nosewheel became embedded in a hole. Fire, the ignition source for which has not been determined, broke out among electrical systems in the fuselage near aft cabin bulkhead, causing circuit breakers for multiple systems to trip and some smoke in the cabin. By the time the aircraft had landed, with no operating radio transmitter, and parked the aircraft, the flames had made a small hole in the fuselage and ground crew called the airport fire services. Ditched in darkness having run out of fuel, but pilot had alerted the coastguard and was picked up. Directional control lost upon selection of reverse thrust. Nosegear collapsed during landing roll. Crashed soon after a night takeoff at St Vincent Airport. At about 100ft the aircraft rolled left and plunged into the sea some 300m from the end of the runway and slightly left of the extended centreline. Just after a night VMC takeoff, at about 100ft the aircraft began a left turn from which it entered a descent until impact. The left engine reduced power during the take-off run and the aircraft veered off the runway. It became airborne aid landed back on the runway about 1.600m from the start of the takeoff run. Aircraft hit Cerro de las Mitras in poor weather and fog, about 20km from the Mont rtey VOR.V.I- e descending inbound to Monterrey. Crashed on runway during an attempted singleengined go-around during crew training. Nose undercarriage collapsed on landing. En route from El Paso, Texas to Toluca. Mexico, the pilot asked to climb from FL370 to FL410 to avoid thunderstorms. Appears to have lost control soon after. Ran out of fuel on final approach after a journey close to aircraft's maximum range, during which the pilot had to leave track to avoid storms. Pilot judged the aircraft to be in danger of overrun on a wet runway, so initiated a go-around. The aircraft overran anyway and collided with trees. The nosegear failed to extend, so the pilot elected to land without it. The failure was in the landing-gear door mechanism. Landed with gear up. Blade on right propeller broke away. Pilots kept control and landed safely. Left main gear reported to have collapsed on landing. FLIGHT INTERNATIONAL 3 - 9 June 1998 35
