Ferry ER Ferry ER AWF MT CT MH Circumstances platform. The aircraft pitched nose down, spun through 360° and rolfed over, striking a Bell 212 (4X-BJN) in the process. Low fuel warning light for the right fuel tank came on. As the pilot responded, the engine failure warning light for the right engine illuminated. The pilot ditched in the sea. Tail rotor struck ground, the heiicopter went out of control and rolled over. Flew into a power line while hover-taxiing above a cherry orchard to allow its downwash to dry the trees. In the pre-ftight site survey the pilot had not seen the wires and the orchard owner did not know they were there. After a rolling takeoff there was a bang apparently from the left engine, then one apparently from the right engine. The helicopter turned right, lost height and hit the ground, roiling over and burning. About 2min after take off on a post-maintenance test flight trie engine reportedly lost power, and the pilot force-landed the helicopter in a marsh. During the approach, the helicopter suffered a main rotor overspeed followed by a loss of engine and main rotor rpm. The heiicopter lost power and undershot its landing site. Hit cable 80ft AGL on approach to a quarry where it was to collect two injured people. Very hard touchdown followed. A main rotor blade separated catastrophically and the helicopter crashed from cruising flight. The blade had failed a blade indicator monitor (BIM) test the previous day but followHjp BIM tests had failed to confirm it or find the cause. The FAA rule is that if a cause is not found the blade must be changed before the helicopter flies again. Pilot elected to make a precautionary landing because of heavy rain and thunderstorms. After a short time, with darkness approaching, the pilot decided to try again even though triere was little change in the weather, but the aircraft crashed soon after lift off. Hit power lines during a second pass over a farmhouse which the helicopter was dispatched to check for fire. Weather CAVOK. While turning from one spray run for the next trie helicopter went out of control and crashed. External toad work. Working for the US Forest Service, the helicopter was being used for fire suppression. After filling the water bucket from a pond the pilot found that he could not lift it, so he backed the helicopter for a second attempt. The cable snagged on the rear of the left skid and when the pilot attempted to lift the bucket again the helicopter rolled left and its main rotors hit the water, causing a crash and roll over. During logging, the cable reportedly broke and flew up into the tail rotor. The pilot force-landed trie helicopter. On approach to a mountain landing site at 5,575ft, the pilot could not slow the rate of descent and it touched down very hard. The day was hot and trie aircraft may have been overweight for the conditions. Hit power lines in daylight transit. The pilot had been called from his planned route to attend an accident and was cleared to resume. Riming 260ft above the sea, trie helicopter lost power and began to lose height. Floats were deployed but the helicopter touched down hard and rolled over. Accident attributed to the unintentional cut-off of fuel. On arrival at a road accident, the pilot elected to land on the road, but dust blew up and obscured his vision. The aircraft touched down hard on one skid which failed, and the helicopter rolled over and caught fire. The left engine is reported to have failed shortly after take off. The pilot made an emergency landing on a road but touched down hard. Hovering at 50ft just before touchdown, the pilot lost control and the helicopter crashed and burned. At trie end of a pipeline survey flight on which the pilot had boarded three unauthorised passengers, the pilot pitched the nose up to reduce speed but the rate of descent increased. The helicopter touched down hard, bounced, hit the ground again, rolled over and burned. 9 After lift off from a remote helipad at elevation 5,600ft, the pilot turned trie helicopter through 180" to leave downslope, but toe aircraft began to sink and the rotor speed to decay. The pilot made a forced landing. The turn downhill was also downwind. There may have been a downdraught. 2 Lost power and crashed during spraying, possibly through fuel exhaustion. 1 Cruising at 2,500ft, the engine suffered fluctuations in.oil pressure and torque, so the pilot carried out a precautionary autorotative landing. The aircraft touched down hard and the main rotor severed the tail boom. The engine had been undergoing maintenance because of metal in the oil, and some worn bearings had been replaced. 2 Cable strike in a remote valley, in daylight but with showers and low cloud base. Heiicopter went out of control and crashed. 1 Loss of power followed by engine surges while filling a water bucket for firefighting. Pilot went for an emergency landing but failed to drop the long line, which caught in frees, pitching trie helicopter nosedown before it crashed into a river. 1 While manoeuvring in the hover the helicopter yawed to one side and rolled over on the ground.
