The pilot found that the cable would not release immediately, but it eventually did. The helicopter landed on the lake shore and its main rotor hit trees.No 1 engine suffered total power loss. Flying downslope, the pilot heard a bang and saw something separate from the aircraft. The helicopter began to rotate and the pilot carried out a forced landing. It is believed that the tail rotor assembly had separated. Intending to carry out single engine landing practice, the flight engineer shut down the wrong engine, forcing autorotation which ended in a landing that wrote the helicopter off. Landed on a hill at 4,000ft elevation. As the pilot reduced power to shut down there was very heavy vibration which caused serious damage despite emergency shut down procedures. Reported separation of both main and tail rotor in flight. While hovering before landing the helicopter developed a high sink rate, touched down heavily and rolled over. Flew into a hill in daylight having, say witnesses, begun a shallow descent at fairly high speed. Suffered an in-flight break up and crashed in fields during approach. Skids snagged on an irrigation pipe during take off from beside the support truck at which the aircraft had reloaded and refuelled. The aircraft nosed over and crashed. The engine failed mechanically while the pilot was manoeuvring to set the 500kg external load down. The helicopter crashed. Descending through 195-2308 at 32kt, the rate of descent increased rapidly and the helicopter hit trees. During a fire suppression flight the aircraft flew along a ridge trailing a bambi bucket which then hit trees. The long line failed and flicked up into the tail rotor, causing its gearbox to fail. The helicopter went out of control, crashed near the ridge and rolled down the slope. Lost power en route and the pilot started an emergency landing approach to a river. During the landing flare the helicopter hit a rock and crashed. On approach there was a loud bang and the helicopter began to rotate to the right, then went out of control and crashed. Control was lost in the last 20ft of an approach to the fire camp, and the helicopter touched down hard and rolled over. LR IE 2E 3 AWE MT - 1 Cruising at 3,000ft, the helicopter began to turn right and pitch down. There was a very hard touchdown from the emergency approach to land. On take offf from a mountain helipad at elevation 9,200ft, the helicopter began to spin and rolled over. There was a light quartering tailwind and it is possible that the tail rotor lost effectiveness. The helicopter was on its sixth long-line logging lift when the aircraft pitched down upon taking up the load. It rapidly lost height and crashed. In normal cruise flight the helicopter suddenly yawed and the end of the tail boom separated. The pilot regained control andmade a forced landing. Damaged in a hard landing. Hard touchdown and main rotors suffered damage. Sling load hit a ridge and was flung up into the tail rotor, which caused loss of control and a crash. Partial power loss after water pick-up in daylight VMC. Helicopter was damaged in the forced landing Main rotors struck the surface of the Amazon. Helicopter crashed. Helicopter crashed into the ground while flying up a narrow mountain ravine at very high altitude. Just before touchdown the pilot elected to set down closer to the picket points and carried out a 90" turn. The nose pitched down, and the pilot reacted by pulling fully back on the cyclic control. This caused the helicopter to back up and climb to about 10m, and it lost main rotor rpm. The aircraft nosed down, banked right and began descending until the main rotors hit the ground. It was operating in a crosswind with an extreme forward CofG. The engine lost power during cruise. The pilot autorotated to land but touched down hard and the main rotor hit the tail. Practice autorotation ended in a hard landing. During a practice autorotation, the helicopter collided with an airborne Robinson R22 light piston helicopter, its ieft skid hitting the R22's main rotor. The 407 landed safely but both occupants of the R22 were killed when it crashed out of control. The engine shut down while in cruise at 1,000ft. A restart attempt failed. The pilot released the sling load and attempted a forced landing on a gravel bar in the river, but the touchdown was very hard. Although there was found to be fuel in the tank it was discovered that the fuel gauge gave a much higher reading than the reality. The pilot appears to have lost control in a crosswind landing, and the helicopter's main rotor struck the ground. The aircraft rolled over. As the helicopter prepared to pick up a load of logs, it spun to the right, 44 FLIGHT INTERNATIONAL 21 - 27 April 1999
