Engine failure following turbine maintenance which entailed disconnection of the pneumatic and fuel lines. The latter were incorrectly reassembled. This apears to have been a misjudged autorotative landing during training. The helicopter crashed and burned. Sudden loss of control en route. Shortly after lift-off. says the pilot, the helicopter kept banking and he could not stop it. Loss of control. On a search and rescue mission the pilot found a missing person on a frozen lake, but while positioning for landing on the lake the pilot became disorientated because of "whiteout" and lost control. Crashed into sea at night in fog. Filming a train, the aircraft hit powerlines above the track and crashed. A clogged fuel filter on this new aircraft led to engine surge. After autorotation the skids dug in and the aircraft nosed over. A bang followed by yaw in the cruise led the pilot to carry out an autorotative landing. Wreckage indicates that part of the tail boom with the tail rotor had separated. Passengers were killed/injured by the main rotor when they disembarked. The pilot lost control during an attempt to take off from an ice covered road on which the helicopter had begun to slide backward. The tail rotor struck an obstruction. The pilot uplifted AvGas instead of Jet Al from a remote refuelling point because the Jet Al hydrant failed to work and his company said AvGas would suffice for a short flight. The engine lost power at 500ft. It appears the AvGas accelerated the breakaway of fuel line particles which clogged the filter. A hard autorotative touchdown caused serious damage. Having picked up skiers, the pilot lost control on take-off because of a whiteout. The helicopter hit the ground and rolled over. Collided with aerial ropeway and lost control. Helicopter suffered reported engine failure as the aircraft was accelerating through 80kt at 150ft AGL carrying an underslung bucket. Pilot jettisoned the bucket and began autorotation. but landed in trees short of the clearing he was aiming for. Temperature 31°C in good weather. Aircraft was operating at close to MTOW. On approach to a landing site the skid snagged sloping terrain in shadow. Pilot lost control and helicopter rolled over. Missing. CNK. Surveying sea ice ahead of an icebreaker ship, pilot reported that because of increasing wind he was going to make a precautionary landing. The next day the aircraft was found to have crashed. After taking off after inspecting an abandoned vehicle, the pilot started to fly up a valley, but found himself in IMC and began a climb, but temporarily lost control. He saw a ridge ahead too late to avoid hitting it with one of the skids and crashed. Helicopter began to yaw as it passed trees on approach for reloading for the next spray run. Control was lost, possibly because the spray boom had snagged the tree, and the aircraft crashed. Flying at 500ft AMSL over the sea from Green Island to Cairns, the pilot, following channel markers, was forced down to between a pressure altimeter height of 50ft and 100ft by deteriorating weather. The windscreen began to mist and the surface was calm. The aircraft hit the sea with little speed. The floats deployed but the helicopter overturned. Right engine failed on approach. The crew landed on a snow-covered field and came to rest on its side. The helicopter turned back on encountering low cloud and deteriorating visibility and climbed to avoid high ground. The crew lost sight of the ground and crashed into a hillside. Taking off from the flatbed trailer of a truck, the engine lost power and the main rotors struck the truck's cab. The aircraft hit the ground with 20° nose down and 30" bank shortly after entenng a snow shower. While landing to pick up skiers, the rotor downwash blew up a large amount of snow, the helicopter drifted right and the skid snagged the ground, causing it to roll over. Taking off from an oil rig into a 2ft hover, the pilot reported pitch and yaw which control inputs could not correct. The aircraft fell off the platform. During final approach, the aircraft, in poor visibility, struck the ground. Struck lightning conductor wti le on power fete Inspection and crashed. Because of deteriorating weather, the pilot elected to make a precautionary landing on a frozen lake. Having set down the external load on the lake, the aircraft began to yaw and it crashed. A student pilot wanted to wash hishelicopter which was parked in his garden. Because the hose would not reach, he decided to hover-taxi the aircraft about 5m.
