The spray deflector appeared to have been damaged on the ground and led to the gear jamming. 5 September British Airways Boeing 777-200ER (G-VIIK) Denver intl apt, USA -/- 15/10 G High-pressure fuel began to spray from the hose/aircraft coupling during refuelling, and a serious fire started. The fire engulfed the refuelling vehicle and damaged the aircraft before fire services put it out. Passengers were still disembarking via the jetway as this occurred. One of the refuelling crew was badly burned. -/- 10/106 -/- 5 September Uzbekistan Airways Tupolev Tu-154M (UK-85576) Ufa apt, Russia The right main gear failed to retract, and when the crew returned to land the right gear collapsed. 6 September Aeropostal MDC DC-9-51 (YV-43C) Port of Spain, Trinidad Nose-gear was torn off while taxiing at night on a taxiway system that was under repair. 16 September VARIG Boeing 737-200 Adv (PP-CJN) Goiania apt, Brazil -/- Bounced on landing and ran off the left runway edge, destroying the nose-gear and right main gear, and tearing off the right engine. 21 September Aeroflot Russian Int'l llyushin II-86 (RA-86074) Dubai apt, United Arab Emirates -/- Landed with gear retracted. 17 October Pakistan Int'l Awys Airbus A300B4-200 (AP-BCJ) Dubai apt, United Arab Emirates -/- Reported that right main gear collapsed on landing and the aircraft veered off the right side of the runway. 21 October Kyrghystan Airlines Yakovlev Yak-40 (EX-87470) Osh apt, Osh, Kyrghystan -/- The aircraft would not rotate, so the pilot abandoned the take-off attempt but the aircraft overran the runway and came to rest in a ditch. 1 November Kenya Airways Boeing 737-300 (5Y-KQD) Nairobi Int'l apt, Kenya -/- 7/41 L The aircraft touched down on the left of the runway in rain following a night ILS approach, and veered further left, leaving the runway. The pilot brought the aircraft back onto the runway, but the nose-gear had been damaged and the engines had suffered ingestion damage. _______ 16 November Middle East Airlines Airbus A321 (F-OHMP) Cairo Int'l apt, Egypt •/• 7/81 L Following a daylight ILS approach on which the aicraft had diverged both above and below the glideslope, which had led the pilot to make arge manual corrections, the aircraft suffered a hard tail-strike on landing, causing serious damage. Passengers disembarked normally at the gate. 20 November EVA Air Boeing MD-11 (B-16101) Taipei Int'l apt, Taiwan -/- 15/208 L The co-pilot landed the aircraft very hard, and the captain took control and went around again. The aircraft landed safely after a circuit but suffered serious damage. 17 December Rio Sul Boeing 737-500 (PT-SSI) Belo Horizonte, Brazil -/- ?/? L Scheduled from Santo Dumont airport, Rio de Janeiro, to Pampulha, but diverted to Belo Horizonte because of bad weather. The left main gear collapsed on landing. ?/? 5/62 15/307 11/193 5/33 TO NON-FATAL ACCIDENTS AND INCIDENTS NON-SCHEDULED PASSENGER FLIGHTS 18 January GT Air DHC Twin Otter (PK-LTZ) Irian Jaya apt, Indonesia -/- 2/3 L The crew could not select reverse thrust on landing, and lost directional control following heavy braking. The right wing struck the ground, but the aircraft was finally brought to a halt on the runway. In subsequent ground tests no fault was reportedly found with the reverse pitch system. 15 March CityBird Boeing 767-300ER (OO-CTR) nr Puerto Plata, Dominican Rep -/- 127/200? C Climbing through FL310 the crew felt a strong vibration, and shortly afterwards the aircraft began to depressurise, so they returned to Puerto Plata. They found that the left off-wing escape slide and the slide compartment door were missing, the inflation bottle discharged, and there were small holes in the left side of the fuselage aft of the wing. It is believed that as the slide detached itself, causing fuselage damage. 3 March AirTransat Boeing 757 (C-GTSF) St John's, Newfoundl'd, Canada -/- 8/175 L The aircraft met turbulence on final approach to runway 11, and touched down hard in a strong, gusting crosswind from the right. The gear was inspected but no damage found. The aircraft flew more flights before the left main gear failed to retract after take-off, and damage consistent with a hard landing was discovered. 15 March CityBird Boeing 767-300ER (OO-CTR) nr Puerto Plata, Dominican Rep -/- 12/200(est) C There was a momentary strong vibration while climbing through FL310, but no other indications of problems until the cabin altitude warning sounded. The crew turned back to Puerto Plata with reducing cabin pressure. The left wing escape slide and its comparment door were missing, and the inflation bottle discharged. 23 March Luxor Air Boeing 707-320C (SU-BMV) Roberts apt, Monrovia, Liberia -/- 7/175 L Just before night touchdown, the aircraft, carrying Haj pilgrims from Jeddah, entered a fog patch and the pilots lost sight of the runway. The approach continued and the touch down was hard, causing the aircraft to bounce. The second touchdown was on the left side of the runway with the right-wing low, and both right engines were torn off. The aircraft ran off the left of the runway, then crossed it again and came to rest near the terminal building. 10 May Spanair Boeing MD-83 (EC-FXY) Liverpool, UK -/- 6/46 Right main gear collapsed during landing roll. Fatigue crack in gear oleo found by investigators. 14 June Cess Air llyushin 11-18 (3C-KKL) Jeddah apt, Saudi Arabia •/- 5/110 A tyre burst and fire broke out in the main gear causing damage before it was extinguished. 25 June Avtex Air Services Embraer Bandeirante (VH-OZG) Cootamundra, Australia -/- 1/9 Starboard generator tripped off line. When the pilot reset it later the engine fire warning light came on, and the pilot elected to shut the right engine down and put the fire out with the extinguisher. He began a diversion toward Young, but the fire warning illuminated again. Eventually smoke began to enter the cockpit, and the weather at Young prevented diversion, so the pilot routed to Cootamundra, isolating all the starboard electrical systems. Cootamundra airport was free of fog, and the pilot landed there safely despite the right wheel tyre failing because of heat damage. The fire was put out by the fire services. 4 August Royal Airlines Boeing 737-200Adv (C-FRYG) Cayo Largo del Sur, Cuba • 4/107 C The crew shut down No 2 engine when oil pressure dropped. Then a No 2 engine fire started and discharging both bottles failed to extinguish it. The crew made an overweight landing and the fire services put the fire out, but it had by then done considerable damage. 19 August Hamburg Int'l Boeing 737-700 (D-AHIA) Ataturk apt, Istanbul, Turkey • 6/135 L Landing on a wet runway the aircraft did not decelerate at the expected rate, and it overran the runway onto the stopway. Some stones on the stopway damaged engine No 2 and its cowl and thrust reverser. 24 August AirTransat Airbus A330-200 (C-GITS) Lajes apt, Azores Islands - 13/291 L A fuel leak downstream of the starboard engine low-pressure fuel cut-off valve was venting fuel to atmosphere at a high rate, and because the crew were crossfeed ing between all tanks the whole system eventually emptied and the engines both stopped within minutes of each other. The ram air turbine provided basic power and hydraulics, and the crew glided the aircraft from about 32,000ft to make a flapless landing on Lajes runway. All tyres burst and the fire service put out wheel fires. The leak had come from a badly refitted fuel feed pipe which had chafed against a hydraulic line since the engine was remounted only a few days before. TO NON-FATAL ACCIDENTS: REGIONAL AND COMMUTER AIRLINES 15 January Uni Air Bombardier Dash 8-300 (B-15235)Shang-Yi apt, Kinmen, Taiwan -/- 4/23 L The aircraft touched down on runway 06 so hard that the main gear collapsed. It was daylight and the wind was 040°/15kt gusting to 25kt, visibility 7km. 18 January Air Nostrum Fokker 50 (EC-GRZ) Barajas apt, Madrid, Spain -/- -/- G A groundcrew put pins in the landing gear because they had to move the gear selector lever up during ground maintenance. The flight crew arrived while the engine neers were still working on the aircraft. The co-pilot noticed the pins during his walk-around check and removed them, then boarded.
