Civil helicopter safety Operation Phase Fatalities Injuries POB 20 206L3 CC-CIV Alas Agricolas Valle Nevado, Chile ? LR Touched down hard on landing at the Valle Nevado skiing resort. 23 206L1 N2138Y EMS Air Service of New York NrSeneca Falls, NY, USA A maintenance error in reassembling the aft short-shaft Thomas coupling had caused loss of tail rotor effectiveness as the pilot started an en route climb from 1,000ft tc 1,500ft. There was not enough aircraft energy for the pilot to achieve the chosen set-down point and the helicopter hit trees on its final autorotative approach. 23 A109A N293G Niihua Helicopters Port Allen Apt, Hawaii, USA - • 2 The aircraft crashed from a low hover during a check sortie. The FAA check airman was criticised in the report for requiring the examinee to carry out, without briefing or demonstration, a manoeuvre that is not a test requirement, namely a "stuck pedal" or anti-torque system failure approach and landing. 23 369D ZK-HMN Milford Helicopters 20km SW of Te Anau, New Zealand AWH MT - 1 The engine failure warning light came on while the pilot was positioning for an approach to set-down in a river valley, and the aircraft settled into trees. It was daylight VMC 25 UH-1H N56CF Vertol Systems Nr Zuni, New Mexico, USA Ferry ER - - 1 The pilot noticed high oil temperature during the cruise, and another helicopter pilot reported that his engine appeared to be on fire. The pilot shut down the engine and carried out an autorotation to a hard landing. The fire went out during descent. 29 206B 4X-BJF Chimnir Aviation Services Nr Lake Kinnenet, Illinois AWH MT Hit power lines and crashed out of control. 3 OH-58 N921MC Hernando Jet Center Nr Lakeland, Florida, USA Test ER 1 In a post-maintenance test flight, the engine power failed and the aircraft was damaged in a hard landing from an autorotative approach. Inspection discovered that the fuel valve switch, selected off during maintenance, had not been switched on again. The pilot's "thru-flight" pre-flight checklist was at variance with the manufac turer's operating manual in not including a check of the fuel valve in pre-flight checks. 7 Mi-8MTV-1 VN-8415 SFC Vietnam Bo Tranh district, Vietnam DNP ER 16 16 The aircraft flew into a hill in bad weather with poor visibility. 8 206B3 N17HA Unknown Ringgold, Virginia, USA Pvt LR 2 The pilot, the owner of the helicopter, was approaching set-down in a garden on a 7° slope, hover-taxiing up the slope on approach. He had set down there on a previous occasion, but in a smaller Hughes 269. While setting down across the slope, the helicopter began to pitch back and forth as the right (uphill) skid touched down. The pilot lifted off again, but the helicopter began to nose down and approach some trees close ahead, so the pilot set the aircraft down. On touchdown it pitched forward and then rolled on to its left side. 12 OH-58 N91335 Beach Mosquito Control Panama City, Florida, USA AWC MT - 1 Attempting to turn left at an IAS of about 35kt for a spray run, the helicopter yawed right about 30° and the pilot could not prevent it. The pilot said he had lost tail rotor effectiveness in the turn possibly because of the turn relative to a local wind-speed of 10-15kt from the south east. 18 369D N713HT Aerial Solutions Nr Du Quoin, Illinois, USA AWE MT - - 1 During tree-trimming operations using a suspended saw the helicopter's engine reportedly failed and the aircraft was destroyed in the hard landing. 22 212 C-FAHZ Alpine Helicopters SW of Golden, British Columbia, Canada AWP LR - - 13 Main rotor struck a rock face as the helicopter landed to set down a party of heliskiers. Control was maintained and the helicopter remained upright. Weather and visibility were adequate. 23 206L3 N215M Critical Air Medicine Nr Wilcox, Arizona, USA Pvt The helicopter lost power in the cruise at 500ft, but it hit a tree just short of the pilot's intended landing area. 25 AS350B PT-HNO Elipidio Marchesi Jr. Jacarepagua Ap, Rio De Janeiro, Brazil CT While simulating a hydraulic failure on the ground, the aircraft went out of control and rolled over. 25 206B PT-HXZ Tulio Mecene Barueri, Sao Paulo, Brazil Pvt Suffered a loss of engine power just after take-off and looked as if it was turning to return to the helipad. It crashed into a building about 500m from the helipad. 27A109AII D-HAZI Rotorflug Nr Arnsberg, Germany Ferry ER 1 The aircraft flew into gently rising ground about 7km from its destination. It was daylight with good visibility. 28 369E C-FMHM Helimax 49km N of Baker Lake, Nunavut, Canada DNC ER - 1 En route there was a loud bang and the cyclic control began to vibrate, followed by another bang after and the pedals began to vibrate. The pilot entered autorotation, but just before touchdown the aircraft entered white-out in blowing snow. The helicopter rolled over. ER GR Tl 5 206 D-HFKB Rhein-Ruhr Helicopter Crashed immediately after take-off. 8 SA365N VT-? Pawan Hans Helicopters Crashed in bad weather. Hameln, Germany Nr Tezpur, India ? The pilot continued the approach but undershot and caught the right skid on the pad edge, causing the helicopter to roll over on to its right side. 18 407 RP-C2345 Jaka Transport Group Sltio Mabugay, Pala Pilot lost control in a forced landing following engine power loss. 19 AS332L1 JA6787 Naka Nihon Air Service Kuwana, Japan CT ? 6 2 Mid-air collision with a Cessna 172 that also crashed, killing the four people on board. 40 22-28 OCTOBER 2002 FLIGHT INTERNATIONAL www.fliqhtinternational.com
