The October 2001 crash of a Terrier Orion rocket carrying the Hyshot experimental scramjet was probably caused by first-stage guidance fin failure, according to the final accident investigation report. The fixed fins broke away during the first minute of flight after support structures gave way. "Examination of the fracture surfaces indicated overload through the fixed fin spindle (journal) sockets," the report says. The Terrier was modified with larger fins from a Nike missile "to achieve the required stability and ensure a stable platform". Hyshot engine developer, the University of Queensland, plans a second launch in late July.
