Australia's Transport Safety Bureau (ATSB) has launched an investigation into the 1 March failure of a Rolls-Royce RB211-524H engine powering a British Airways Boeing 747-400. The failure occurred at FL370 (37,000ft) about an hour after the aircraft departed Sydney for Bangkok, causing severe vibration problems. The crew returned to Sydney and made a safe landing after dumping fuel. R-R believes the number three engine shed a fan blade and the ATSB is carrying out a full accident investigation because debris pierced the leading edge of the wing and made a small hole in the fuselage.
