The opening paragraph of your safety review (Flight International, 20-26 January) says "from controlled flight into terrain as the biggest cause of fatal airliner accidents". CFIT is a result not a cause.

The only definite things you can say about any accident is that there is always more than one cause and that there is always an element of human error. CFIT accidents (events) result from many different causes; loss of situational awareness, poor training and so on. It is not a cause in itself.
